One Side More Swollen After Surgery? Why, and When to Worry

Why swelling is uneven
A few ordinary reasons: your body simply heals at slightly different rates on each side, the position you lie or sleep in lets fluid pool on one side, and small differences in the surgery itself. Most people are also naturally a little asymmetrical to begin with, which becomes easier to notice while you're swollen.
What it looks like by procedure
After a BBL, one cheek can look bigger as fat takes and fluffs at different rates. After a tummy tuck, one side can hold more fluid (and a seroma is usually one-sided). After breast augmentation, one implant often “drops and fluffs” before the other. After lipo, one area can stay firmer or more swollen as it smooths out.
How long uneven swelling lasts
It usually evens out as the overall swelling resolves — weeks for some procedures, a few months for others. A small, permanent difference between sides is common and normal. What matters is the trend: it should be improving, not suddenly getting worse on one side.
Uneven swelling that needs urgent care
Two situations are different from normal unevenness. One calf (or leg) that becomes swollen, painful or warm can be a blood clot — that's an emergency. And a fast-growing, hard or very painful fluid pocket on one side can be a seroma or hematoma — call your surgeon promptly. After breast augmentation, one breast suddenly much larger, firmer or more painful needs a same-day call.
What recovery really feels like
“One side looks bigger than the other” is one of the most common post-op panics, and the vast majority of the time it's just normal, uneven healing that evens out. The real job of knowing this is recognizing the rare times it isn't — so you can tell the difference between “be patient” and “call now.”
Frequently asked questions
How long is it normal to be swollen after surgery?
It depends on the procedure — weeks for the big swelling, and up to several months or a year for the last of it. Uneven swelling usually evens out along the way.
What day is swelling the worst after surgery?
Usually around days 3–5, then it slowly improves.
What does asymmetrical swelling mean?
Most often it just means one side is healing or draining a little differently and will even out. Sudden, hard, hot or painful one-sided swelling is the exception that needs a call.
Why is one breast bigger after augmentation?
Early on, one implant often settles and softens before the other; it usually balances out over weeks to months.
